Hospice: What Does It Mean? What Can We Expect?
Rebekah Capen, social worker for Home Health & Hospice, Foundation Health Partners, will navigate the discussion surrounding hospice. "It's an interdisciplinary approach to care. It includes nursing staff, CNAs, a chaplain or spiritual advisor, social workers, our volunteers, and a medical director." The team creates an individualized plan of care based on the needs and goals identified by the clinical team and the patient.
